VERIFICATION OF AFFILIATION

You may verify provider affiliation with Carteret General Hospital on our secure portal. Please follow the following steps:
- Go to http://tiny.cc/CGH
- Select Hospital Verification Letter (If you are unable to view the full page, click Tools on the toolbar, Compatibility View)
- Enter the last name of the practitioner you are querying
- Select the name of your facility
- Select the practitioner
- Enter your name as the requestor
- Click View Report
- Print the verification letter
If you are unable to find the practitioner, please email your request to mss@ccgh.org with the appropriate signed authorization to release. Thank you.
